Billing and Cancellation: 

I do not bill insurance companies directly.  A nutrition service bill can be provided including diagnosis and CPT codes which can be submitted to your insurance carrier.

Appointments can be made by calling (801) 920-5400 or emailing wsueatrite@msn.com.  Payment is expected at time of service.  If you are unable to keep an appointment, please give 24 hours’ notice or a cancellation fee will be made for the time reserved.  I do make exceptions for emergencies, driving conditions or sudden illness.

I offer nutrition counseling online or in person in my South Ogden, Utah home office.
